Metabolic mechanisms of chemiluminescent activity of neutrophilic granulocytes in qualified athletes

Abstract
The high exercise stress in sport causes the decrease of immune cells reactivity. Studying the interrelation between hemiluminscent activity of neutrophylic granulocytes and NAD(P)-depending dehydrogenases in athletes is important for better understanding of sport immunodeficiency metabolic mechanisms and researching the methods for its correction. 51 male athletes of 22,1 ± 3,7 y. o. and 64 healthy males of the same age as the reference group were participated in the study. We found that metabolic resources providing the intensity of respiratory burst depend on mitochondrial processes condition and lipid metabolism. The activation index has logarithmic dependence on NAD(P)-MDH, NAD-MDH, NAD-IDH activities. Therefore the functional activity of neutrophilic granulocytes in athletes has positive correlation with plastic exchange level and negative correlation with oxygen-depended ATP production.

Abstract
The study included 71 patient with ischemic coronary disease (ICD) with type II diabetes mellitus (DM) treated by oral hypoglycemic agents undergoing off-pump coronary artery bypass grafting (CABG). The mean age was 59 (56-64) years. Before the surgery the patients were randomized to continuing of oral hypoglycemic agents (n = 34) or to switching to rapid-acting insulin (n = 37). In the studied groups we compared glycemic parameters, markers of myocardial necrosis (troponin T and creatinine phosphokinase-MB (CPK-MB)) and acid-base balance, as well as their dynamics during the perioperative period. The relationships of myocardial necrosis markers dynamics with glucose metabolism and acid-base balance parameters were investigated. Before the surgery in switched to rapid-acting insulin patients serum glucose, its daily variability and the proportion of blood glucose values less than recommended perioperative range (6,1-10,0 mmol/l) were higher than in those at oral hypoglycemic agents. The myocardial necrosis markers at the day before and 24 h after surgery as well as lactate level and blood acidity during and at 24 hours after surgery were higher in the insulin-group. The insulin-group was characterized by greater increase of troponin T during 24 h after surgery (0,36 [0,25-0,40] vs 0,2 [0,1-0,25] ng/ml; p < 0,001) and CPK-MB (14 [8,0-17,0] vs 7,3 [1,5-8,7] U/L; p < 0,001). Troponin T and CPK-MB dynamics correlated with serum glucose during surgery (r = +0,81 and r = +0,66 respectively), daily serum glucose variability at the day before surgery (r = +0,71 and r = +0,63) and the first day of admission to hospital (r = +0,37 and r = +0,31).

Abstract
From 2009 to 2014 total hip replacement on the account of posttraumatic coxarthrosis of the of the 3rd degree in combination with coxal cavity defect, dislocation or incomplete dislocation of femoral head was conducted in our clinic in 12 patients (10 (83,3 %) men and 2 (16,7 %) women) of 23-51 years (42 on the average). It took from 3 weeks to 8 years (26 months on the average) from the moment of trauma till the operation. We used the method of the replacement of defect of posterosuperior edge of coxal cavity of femoral head. We used acetabular components of cementless fixation in 3 (25 %) cases and mounted a cup of cement-retained fixation in 9 (75 %) cases. The operation lasted from 45 to 225 minutes (90 minutes on the average). Blood loss was from 500 to 2100 ml(1100 ml on the average). 3 (25 %) patients had complications in postoperative period. 2 (16,7 %) patients in consequence of noncompliance of orthopedic regimen had dislocation of prosthesis head on the 8th and 14th day after the operation. 1 (8,3 %) patient had soft tissues infection in the zone of operative intervention in postoperative period. Terms of observation of alteration of massive autografts and functional results of hip replacement were from 1 to 5 years. 11 (91,7 %) patients didn't have any signs of acetabular component instability and osteolysis in the zone of contact between bone cavity and cup. No patient had signs of instability of stem of joint. Harris score increased from 34,1 ± 7,2 to 78 ± 8,4 points.

Abstract
It is known that research of immune system using separate parameters doesn't allow to estimate it as one. The aim of the work was to find new approaches to the estimation of immune system state for the determination of character and intensity of immunopathological manifestations in patients with hip joint pathology. We examined immune status in 73 patients with degenerative-dystrophic injuries and in 18 patients with posttraumatic hip joint injuries. We found the most significant immunological indices that allow to determine the state of immune reactivity of the patient before the operation. We identified two groups of patients: with presumably "compensated" and "subcompensated" form of immunodeficient state. It was shown that infectious infectious complications in the area of endoprosthesis appear in patients with low immune reactivity on the basis of calculation of equations of linear function in patients with posttraumatic coxarthrosis. The results of the research showed that it's not necessary to analyze many immunological indices to estimate the state of immune reactivity of an organism. Using discriminant analysis allows to reveal the determinant factors that characterize distinctness of Junctional state of immune system of a certain patient and to determine the risk of development of postoperative complications.

Abstract
According to the results of multi-layer spiral CT, the main reason of shoulder instability is a bone defect of scapula articular surface. Frequency of poor results after common surgical treatment is high and runs up to 30 % of cases. The aim of the research was to estimate long-term results of surgical treatment of patients with chronic posttraumatic anterior shoulder instability at the edge bone defect of scapula articular surface using restoration of the anatomy of scapula articular surface by free autograft. We conducted open controlled prospective randomized study in the clinic of Irkutsk Scientific Center of Surgery and Traumatology. Estimation of long-term results of surgical treatment was conducted by the next criteria: functional (range of motions of operated upper extremity), subjective (visual analogue scale of pain, integral estimation of functional results by Rowe - Zarins and DASH scales), objective (X-ray imaging of shoulder joint in -x views and multi-layer spiral CT) and complications. Estimation of functional and anatomic results of 34 patients in 3 years after the operation proved clinical effectiveness of proposed surgical method. Overall level of conclusiveness of the research can be defined as level B and the effect of treatment as class I. Therefore, using free autograft from iliac crest allows to restore lost surface of scapula cavity more than 15 % of total surface.

Abstract
The research was based on the data of 155 newborns with manifestations of cytomegalovirus infection (CMV) appeared in the initial few months of life and on the results of the 7-years follow-up of this group of patients. While studying the courses and outcomes of CMV appeared in the initial few months of life in 155 patients of the tender age the disability is set for 22 (14,2 %) children. On the basis of discriminant analysis with the use of the most significant factors the model of prognosis of the outcomes of the carried infection was created in severe residual-organic disorders, dictating the necessity of determination of disability for children. This model can predict development of severe organ pathology in children of tender age after citomegalovirus infection appeared in the initial few months that will allow to select patients with high probability of disability and to realize for them individual rehabilitation measures to save their health in proper time.

Abstract
Visual researches showed that the cancellous bone of the distal radius has significant morphological changes with the increase of years. Revealed structural changes of cancellous bone allows to differentiate the age of an object and to direct a medical expert towards the proposed age group at the identification study. As a result of photocolorimetric research of involution changes of cancellous bone of the distal radius we found statistically significant increase of light transmission with the increase of years which is associated with an increase of osteoporosis. The correlation coefficient ranged from 0,63 to 0,80. On the basis of these data mathematical models that allow to determine the person's age in the forensic identification researches of the distal radius were developed (standard mean square error in determination of the age was from 6,6 to 8,2 years).

Abstract
Urinary tract infections are one of the common diseases that are frequent in clinical practice of different physicians. The main agents that are plated from urine at the urinary tract infections are E. coli, Proteus mirabilis, Klebsiella spp., Pseudomonas spp., Staphylococcus aureus, Morganella morganii, Citrobacter freundii. Effective treatment of UTI is only possible with the restoration of normal urodynamics and the correction of complicating factors and concomitant diseases. The aim of the study was to evaluate pharmacotherapeutic efficacy and safety of the medication "Uroprofit" in complex treatment of UTl. At the treatment of persistent bacterial infection of the lower urinary tract which is one of the challenges in urological practice, "Uroprofit" is an effective and safe medication. It has antibacterial, anti-inflammatory, antispasmodic, immunomodulatory properties.

Abstract
In this article we analyzed social efficiency of orthotics department at the Central Municipal Hospital of Ulyanovsk. The study was conducted in the period of 2013-2014 years in patients with different diseases. All respondents of experimental (n = 150) and control (n2 = 150) groups who participated in the study expressed positively about the purpose and use of orthoses - "good" (nt = 63,7 %; n2 = 80,7 %) or "no answer" (nt = 36,3 %; n2 = 19,3 %). It shows a high level of social efficiency of provided type of care. Work of orthotics department will increase the number of orthotic devices used to improve the quality of life of patients.

Abstract
For two groups of Russian teenagers the proportion of deletion (Del) allele of the apolipoprotein-B gene polymorphism (ApoB, Ins/Del, rs17240441) was studied. One group of 70 teenagers with essential hypertension, and the comparison group - 73 healthy teenagers. In both groups, the most common genotype was heterozygous genotype Ins/Del, in the group with essential hypertension it was 67,1 %, in the control group - 52,1 %. The frequency of Del allele in the group with essential hypertension was 47,9 %, which was significantly higher than in the comparison group - 30,1 % (p = 0,003). Thus, the high frequency of deletion allele in the studied group with hypertension indicates an increased risk of cardiovascular disease in this group in the future and has prognostic significance.

Abstract
We conducted an analysis of Russian and foreign literature and an analysis of our researches on the problem of etiology and pathogenesis of viral hepatitis was showed. Viral hepatitis occupies a leading position among infectious disease. Chronic forms of viral hepatitis B and C is the most prevalent. Among them more than 60 % are registered in age from 20 to 39 years. In spite of the fact that pathogenesis, clinical aspects and chronization of these forms are different, they have the general processes in a liver. The changes of these processes of the general systems of an organism (such as the neuroendocrine regulation, lipid status and lipid peroxidation - antioxidant system) have the particular importance. Studying these variables under abnormalities of reproductive health in women with hepatitis is particularly topical.

Abstract
According to the data of World Health Organization, weak sight at uncorrected anomalies of refringence is one of the main directions of liquidation of preventable blindness to the 2020. The article presents the review of present-day literature devoted to the etiology, pathogenesis of myopia and its complications. Data of Russian and foreign researchers are presented. The estimation of the methods of research in myopia diagnostics and associated complications is given. The attention is given to the unsolved problems of early diagnostics of myopia complications.

Abstract
The article presents the result of treatment of patient P. with trauma caused by an attack of a bear during the hunting. Patient had open comminuted displaced fracture of the lower third of right ulnar bone with formation of large infected wound of right forearm and right hand with injury of flexor tendons of IV and V fingers, extensor tendon of V finger, ulnar nerve. At the admission to the Central District Hospital of Mama, primary surgical treatment of wound, plaster immobilization were conducted, wound weren't stitched up. At the treatment in Irkutsk Scientific Center of Surgery and Traumatology, secondary surgical treatment of wound of right forearm, necretomy, ultrasound treatment of wound, laceration repair of wound with local tissues, transosseous osteosynthesis of right forearm - hand were conducted. As a result of conducted treatment we achieved total reduction of necrotic and purulent process, repair of vast infected wound of forearm and hand, full adhesion of fracture of ulnar bone and restoration of a function of right arm. Using ultrasound treatment of wounds and transosseous osteosynthesis at openfractures of forearm bones and chronic purulent infection is a treatment of choice and can cause good clinical outcome.

Abstract
Traumatic isolated injury of deltoid muscle is a rare nosological orthopedic injury that occurs in 1,3 % of all shoulder joint injuries. In normal state function of shoulder joint is provided by deltoid muscle up to 50 % of capacity in scapular area. Degree of abolition of function of shoulder joint depends on the degree of deltoid muscle injury. Differential diagnosis is necessary to conduct with other traumatic injuries accompanied with such upper limb dysfunctions as chronic posterior humerus head dislocation, injury of rotatory cuff tendon, chronic complete or partial disorder of deltoid muscle integrity. The article contains clinical case of diagnostics and surgical treatment of the patient with isolated old deltoid muscle injury of more than 2 years accompanied with combined contracture of shoulder joint. Check-up examination in 6 months after the operation showed full restoration of a function of upper limb and no complaints. Results of control MRT examination of shoulder joint in 11 months after the operation showed full adhesion of tendomuscular autograft with acromial process of scapula.
