Диагностика и лечение грибкового кератита представляет значительные трудности в повседневной офтальмологической практике. Это обусловлено тяжелым, резистентным течением заболевания и высоким риском рецидива, даже при проведении адекватной терапии. Одной из особенностей кератомикоза является способность клеток мицелиальных грибков распространяться за пределы видимой зоны поражения, что может снижать эффективность лечения.</p></sec><sec><title>Цель</title><p>Цель. Определить истинную степень распространения мицелия нитчатых грибков в строме роговицы при кератомикозе.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Исследование выполнено на экспериментальной модели кератита у кроликов путем выполнения многократных интрастромальных инъекции 0,1 мл суспензии изолятов Fusarium solani, выполняемых до формирования четкого грибкового очага. Для оценки разницы между визуальной и истинной границей протяженности патологического процесса использованы такие методы, как биомикроскопия с фотофиксацией, оптическая когерентная томография (ОКТ) роговицы, а также гистологическое исследование. Измерения проводили с учетом анатомических особенностей роговицы.</p></sec><sec><title>Результаты</title><p>Результаты. На 7–9 сутки на 9 глазах из 14 удалось добиться формирования четкого очага грибкового поражения, подтвержденного в дальнейшем путем посева на специфические питательные среды. При проведении гистологического исследования грибковые клетки удалось обнаружить на 17 срезах из 45. При количественной визуальной оценке с использованием щелевой лампы и гистологической картины установлено различие между данными показателями (p = 0,0036), при этом показатели медианной зоны распространения в строме роговицы были больше по сравнению с визуально определяемым инфильтратом. Также во всех образцах обнаружено проникновение грибковых клеток во всю толщину роговицы с выявлением их во влаге передней камеры.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные результаты свидетельствуют, что к восьмому дню развития кератита распространение грибковых клеток вдоль коллагеновых слоев может превосходить зону видимого поражения, а по глубине распространяться вплоть до передней камеры. Эти особенности роста мицелиальных грибков следует учитывать при планировании объема лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. The diagnosis and management of fungal keratitis remain major challenges in routine ophthalmic practice. This is largely attributable to the severe, treatment-resistant course of the disease and the high risk of recurrence, even when appropriate antifungal therapy is administered. A distinctive feature of keratomycosis is the ability of filamentous fungi to extend beyond the clinically visible margins of the lesion, which may significantly compromise treatment efficacy.</p></sec><sec><title>The aim</title><p>The aim. To determine the true extent of mycelial fungal spread within the corneal stroma in keratomycosis.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. The study was performed using an experimental rabbit model of keratitis induced with Fusarium solani isolates. To assess the actual extent of the pathological process, slit-lamp biomicroscopy, anterior segment optical coherence tomography (OCT), and histological examination were employed. Measurements were obtained with consideration of corneal anatomical features.</p></sec><sec><title>Results</title><p>Results. On days 7–9 after keratitis induction, in 9 cases demonstrated a well-defined biomicroscopic appearance consistent with fungal keratitis, with confirmed fungal etiology. Histological examination revealed fungal cells in 17 of 45 sections. A quantitative visual assessment using a corneal microscope and histological examination revealed a difference between these parameters (p = 0.0036), with the median area of proliferation in the corneal stroma being greater than the visually detectable infiltrate. Fungal cells also penetrated the entire corneal thickness in all samples, with detection in the aqueous humor of the anterior chamber.</p></sec><sec><title>Conclusion</title><p>Conclusion. The findings indicate that by day 8 of keratitis progression, the spread of fungal cells along stromal collagen lamellae may exceed the clinically visible lesion by nearly half and extend in depth to involve anterior chamber structures.
