Неоадъювантная химиотерапия (НХТ) является частым выбором для лечения рака молочной железы (РМЖ) у пациенток с трижды негативными HER2-позитивным подтипами. Использование НХТ при HER2-негативном РМЖ является предметом дискуссий. Для пациенток с HR+/HER2-РМЖ выбор НХТ по-прежнему основан на клинических и патологических характеристиках, а не на биомаркерах с определенной клинической ценностью, в отличие от адъювантной терапии, где сигнатуры экспрессии генов широко используются для принятия решений. Следовательно, крайне важно оценить не только эффективность использования НХТ, но и выявить маркеры для прогнозирования течения заболевания в этой конкретной группе пациенток.</p></sec><sec><title>Цель</title><p>Цель. В настоящей работе проведено изучение изменения экспрессионного профиля HER2-негативных опухолей молочной железы при применении таксан-содержащих схем НХТ.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В качестве материала использованы парные образцы биопсии до лечения и опухолевой ткани (после НХТ) для 17 пациенток с РМЖ. Экспрессионный ландшафт опухоли оценивался при помощи полнотранкриптомного микроматричного анализа с использованием микрочипов Clariom™ S Assay, human (Affymetrix, USA). Кривые общей и безметастатической выживаемости для пациенток, включенных в исследование, построены по методу Каплана – Майера. Проведена валидация полученного результата на независимой выборке.</p></sec><sec><title>Результаты</title><p>Результаты. В результате сравнения экспрессионного профиля опухоли пациенток до и после НХТ таксан-содержащими схемами определено, что в опухоли до лечения уровень экспрессии гена PODXL2 в зависимости от ответа на терапию, и в опухоли после проведения НХТ уровень экспрессии генов NR2E1 и TMEM98 в зависимости от статуса гематогенного метастазирования, статистически значимо коррелируют с общей выживаемостью (ОВ) и безметастатической выживаемостью (БМВ) пациентов, соответственно.</p></sec><sec><title>Заключение</title><p>Заключение. Идентифицированы потенциальные экспрессионные маркеры объективного ответа на применяемое лечение и прогнозирование возникновения гематогенного метастазирования опухоли молочной железы HER2-негативного при назначении таксан-содержащих схем лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Neoadjuvant chemotherapy (NAC) is a common treatment option for patients with triple-negative and HER2-positive breast cancer (BC) subtypes. The use of NAC in HER2-negative breast cancer is a matter of debate. For patients with HR+/HER2-breast cancer, the choice of NAC is still based on clinical and pathological characteristics rather than on biomarkers with a certain clinical value, in contrast to adjuvant therapy, where gene expression signatures are widely used for decision-making. Therefore, it is imperative to evaluate not only the efficacy of NAC but also to identify markers for disease prognosis in this specific patient group.</p></sec><sec><title>Aims</title><p>Aims. In this work, we studied the changes in the expression profile of HER2-negative breast tumors when using taxane-containing NAC regimens.</p></sec><sec><title>Material and methods</title><p>Material and methods. The material used was paired biopsy samples before treatment and tumor tissue (after NAC) for each patient. The expression landscape of the tumor was assessed using whole transcriptome microarray analysis using Clariom™ S Assay, human microarrays (Affymetrix, USA). Overall and metastasis-free survival curves for patients included in the study were constructed using the Kaplan-Meier method. The obtained result was validated on an independent sample.</p></sec><sec><title>Results</title><p>Results. As a result, a comparison of the expression profile of the tumor of patients before and after NAC with taxane-containing regimens determined that the expression level of the PODXL2 gene in the tumor before treatment, depending on the response to therapy, and the level of expression of the NR2E1 and TMEM98 genes in the tumor after NAC, depending on the status of hematogenous metastasis, statistically significantly correlate with the OS and RFS of patients, respectively.</p></sec><sec><title>Conclusions</title><p>Conclusions.
