Исследование системы гемостаза при шизофрении имеет высокую научную и клиническую значимость в связи с множественными взаимосвязями между нарушениями свертывания крови, нейровоспалением и патогенезом психотических расстройств. Это направление, находящееся на стыке психиатрии, нейробиологии и гематологии, открывает новые перспективы для понимания патофизиологии шизофрении и разработки персонализированных подходов к терапии.</p></sec><sec><title>Цель</title><p>Цель. Оценка роли системы гемостаза методом «Фибринодинамика» в патофизиологии шизофрении.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено наблюдательное когортное исследование 66 пациенток 16–57 лет (32,2 ± 10,2 года), госпитализированных в клинику ФГБНУ НЦПЗ в острой стадии заболевания. Параметры свертывания и фибринолиза оценивали методом «Фибринодинамика», клиническое состояние — по шкалам PANSS и HDRS.</p></sec><sec><title>Результаты</title><p>Результаты. До лечения отмечено статистически значимое превышение контрольных значений всех показателей «Фибринодинамики», кроме параметра сбалансированности гемостаза (OFP%, p = 0,337), что свидетельствует о высоких потенциалах коагуляции (OCP, p &lt; 0,0001) и фибринолиза (OFP, p &lt;0,0001) при сохранении общей сбалансированности системы. После лечения выявлено нарушение баланса гемостаза (OFP%, p = 0,0394) с возможным преобладанием коагуляционного или фибринолитического потенциала у разных пациентов (OCP, p &lt; 0,0001; OFP, p &lt; 0,0001), при отсутствии статистически значимых изменений показателей в группе в целом.</p></sec><sec><title>Заключение</title><p>Заключение. Показано улучшение психического состояния по психометрическим шкалам и наличие корреляций ряда показателей «Фибринодинамики» с клиническими параметрами, что подтверждает вовлеченность гемостатической системы в патогенез шизофрении. Учитывая связь гемостаза и воспаления, результаты позволяют предположить ассоциацию дисфункции гемостаза при шизофрении с хроническим воспалением как ключевым патогенетическим механизмом.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. A multiple and variable interrelations between the blood coagulation disorders, neuroinflammation and the psychotic syndromes pathogenesis mean an obvious scientific and clinical validity of the hemostasis system research in schizophrenia. This interdisciplinary track deals with a link between psychiatry, neurobiology and hematology. This itself is a step towards the new perspectives for further understanding of schizophrenia pathophysiology, and, consequently, for developments of the forth-coming therapeutic approaches.</p></sec><sec><title>The aim</title><p>The aim. Evaluation of the role of the hemostasis system using the “Fibrinodynamics” method in the pathophysiology of schizophrenia.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. An observational cohort study was conducted on 66 female patients aged 16–57 years (32.2 ± 10.2 years) hospitalized in the clinic of the Mental Health Research Center in the acute stage of the disease. Blood coagulation and fibrinolysis patterns were detected and analysed using our original version of “Fibrinodynamics” method, while the patients` clinical status was elucidated by PANSS and HDRS scales.</p></sec><sec><title>Results</title><p>Results. Prior to treatment, a statistically significant increase of all “Fibrinodynamics” control values, except for a hemostasis balance parameter (OFP%, p = 0.337) has been observed. This shows a high potential of coagulation (OCP, p &lt; 0.0001) and fibrinolysis (OFP, p &lt; 0.0001), whereas the whole system balance was found intact.</p><p>After the treatment, a clear hemostasis disbalances (OFP%, p = 0.394) with a probable predominance of coagulation and/or fibrinolytic potential in different patients (OCP, p &lt;0.0001; OFP, p &lt;0.0001): a lack of statistically essential changes of these parameters was demonstrated within this group in general.</p></sec><sec><title>Conclusion</title><p>Conclusion. The data obtained are in a favor to a statistically significant increase of the hemostasis function parameters in schizophrenia patients which correlates with their clinical status. This indicates to involvement of the hemostasis system into pathopysiology of the disease studied.</p><p>Taking into account the link between inflammation and hemostasis we’ve found, these results allows to assume an essential interrelation between the hemostasis disfunction and chronic inflammation, as long as both takes place in schizophrenia patients.
