С момента создания реверсивного эндопротеза плечевого сустава P. Grammont в 1985 году, показания к его применению значительно расширились. Конструкция эндопротеза была улучшена, что существенно снизило количество послеоперационных осложнений. При этом вопрос о тактике работы с подлопаточной мышцей при реверсивном эндопротезировании до сих пор остается открытым.</p></sec><sec><title>Цель</title><p>Цель. Сравнить функциональные и клинические результаты у больных после реверсивного эндопротезирования плечевого сустава, выполненного с использованием дельтопекторального доступа с полным или частичным отсечением подлопаточной мышцы.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено рандомизированное клиническое исследование результатов реверсивного эндопротезирования плечевого сустава у 49 пациентов. Пациенты были разделены на две группы. В первой группе имплантация реверсивного эндопротеза была выполнена через стандартный дельтопекторальный доступ, во второй группе пациентов был использован разработанный доступ с частичным отсечением подлопаточной мышцы. В послеоперационном периоде, а также через 3 и 12 месяцев после операции, у пациентов двух групп были проанализированы клинические и функциональные результаты с использованием шкалы ASES и ВАШ боли.</p></sec><sec><title>Результаты</title><p>Результаты. Пациенты, которым был выполнен доступ с частичным отсечением подлопаточной мышцы, в послеоперационном периоде демонстрировали лучшие показатели по шкале ASES, больший объем переднего сгибания в плечевом суставе и меньшую интенсивность болевого синдрома по сравнению с группой пациентов, которым был выполнен стандартный дельтопекторальный доступ.</p></sec><sec><title>Заключение</title><p>Заключение. Доступ с частичным отсечением подлопаточной мышцы позволяет улучшить клинические и функциональные результаты лечения пациентов после реверсивного эндопротезирования плечевого сустава, а также способствует снижению вероятности вывиха реверсивного эндопротеза.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Indications for reverse shoulder arthroplasties (RSA) have increased since their development by Paul Grammont in 1985. The design of the endoprosthesis has been improved, significantly reducing the incidence of postoperative complications. However, the question of how to treat the subscapularis muscle (SSC muscle) during reverse shoulder arthroplasty remains open.</p></sec><sec><title>The aim</title><p>The aim. To compare the functional and clinical outcomes in patients undergoing reverse shoulder arthroplasty performed via the deltopectoral approach with either complete or partial subscapularis muscle dissection.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A randomized clinical trial was conducted for evaluating the outcomes of reverse shoulder arthroplasty in 49 patients. The cohort was divided into two groups. In the first group, implantation of the reverse endoprosthesis was performed using the standard deltopectoral approach, whereas in the second group a novel surgical approach with partial subscapularis muscle dissection was employed. Clinical and functional outcomes were assessed postoperatively, and at 3 and 12 months of follow-up, using the American Shoulder and Elbow Surgeons (ASES) score and the Visual Analog Scale (VAS) for pain.</p></sec><sec><title>Results</title><p>Results. Patients who underwent a surgical approach with partial subscapularis muscle dissection, in the postoperative period, demonstrated better scores on the ASES scale, a greater degree of anterior flexion and less intensity of pain compared to the group of patients after standard deltopectoral approach.</p></sec><sec><title>Conclusion</title><p>Conclusion.
