Особую сложность представляют массивные разрывы, определяемые как повреждение двух и более сухожилий или ретракция свыше 5 см, сопровождающиеся атрофией мышц и жировой дегенерацией. Лечение таких повреждений является сложной задачей в ортопедической практике в связи с высоким риском рецидива, частота которого достигает 18–94 %.</p><sec><title>Цель исследования</title><p>Цель исследования. Оценить клиническую эффективность хирургического лечения повреждений вращательной манжеты плеча оригинальным способом пластики аутотрасплантатом из половины сухожилия длинной малоберцовой мышцы.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведен проспективный анализ результатов лечения 20 пациентов, имеющих массивные разрывы вращательной манжеты плеча и получавших хирургическое лечение по разработанному оригинальному способу в клинике ФГБНУ «ИНЦХТ».</p></sec><sec><title>Результаты</title><p>Результаты. Анализ интраоперационных параметров (кровопотеря, длительность операции) и послеоперационного функционального восстановления по шкалам UCLA и ASES показал, что спустя один год у подавляющего большинства (95 %) пациентов констатированы хорошие и отличные результаты. У пациентов была достигнута амплитуда сгибания и отведения в плечевом суставе, составляющая 150–180°.</p></sec><sec><title>Заключение</title><p>Заключение. Разработанный способ пластики дефектов вращательной манжеты плеча с использованием аутосухожильного трансплантата, сформированного из ½ сухожилия длинной малоберцовой мышцы, обеспечивает регресс болевого синдрома и восстановление функции травмированной верхней конечности в более сжатые сроки.</p></sec></abstract><trans-abstract xml:lang="en"><p>According to the literature, the frequency of rotator cuff tendon injuries varies from 22.1 % to 34 %, while only 30 % of patients with diagnosed tendon tears experience pain and limited mobility in the upper limb. Massive tears, defined as damage to two or more tendons or retraction exceeding 5 cm, accompanied by muscle atrophy and fatty degeneration, pose a particular challenge. The treatment of such injuries is a challenging task in orthopedic practice due to the high risk of recurrence, which can reach 18–94 %.</p><sec><title>The aim</title><p>The aim. To evaluate the clinical effectiveness of surgical treatment of rotator cuff injuries using the original method of autografting from half of the tibialis anterior tendon. </p></sec><sec><title>Materials and methods</title><p>Materials and methods. A prospective analysis of the treatment results was conducted in 20 patients with massive rotator cuff tears who underwent surgical treatment using the original method developed at the Irkutsk Scientific Centre of Surgery and Traumatology.</p></sec><sec><title>Results</title><p>Results. Analysis of intraoperative parameters (blood loss, duration of surgery) and postoperative functional recovery according to the UCLA and ASES scales showed that after one year, the vast majority (95 %) of patients had good and excellent results. The patients achieved an amplitude of flexion and abduction in the shoulder joint of 150–180°.</p></sec><sec><title>Conclusion</title><p>Conclusion.
