Исследуемая группа включала 20 амбулаторных больных, у которых на фоне максимальных объёмов базисной терапии сохранялось неконтролируемое течение заболевания. Дополнительно к предшествующей терапии пациентам назначался тиотропия бромид в дозировке 5 мг в сутки. Продолжительность исследования составила пять месяцев, в ходе которых ежемесячно оценивались клинические симптомы, наличие обострений БА, показатели спирометрии, уровень контроля заболевания по опроснику Asthma Control Questionnaire™ (ACQ-5) и качество жизни больных. Установлено, что включение тиотропия бромида в схему лечения вызывает статистически значимое улучшение контроля астмы, снижение индекса ACQ-5 на 48 %. Также отмечена существенная динамика постбронходилатационного объёма форсированного выдоха за 1 с (ОФВ1) и показателей качества жизни. Ни у одного из пациентов за период наблюдения не отмечались обострения заболевания. Однако для достижения терапевтического эффекта требовалась систематическая терапия тиотропия бромидом в течение не менее трёх месяцев. Полученные результаты подтвердили положительное влияние тиотропия бромида на контроль бронхиальной астмы и качество жизни пациентов.</p></abstract><trans-abstract xml:lang="en"><p>The aim of the study was to investigate the effectiveness of tiotropium bromide in patients with severe persistent asthma under conditions of real clinical practice. The study group included 20 outpatients who, despite the maximum volumes of basic therapy, maintained an uncontrolled course of the disease. In addition to previous therapy, patients were given tiotropium bromide 5 mg per day. The study lasted five months. During this period, clinical symptoms, the presence of exacerbations of asthma, spirometry parameters, the level of disease control according to the questionnaire Asthma Control Questionnaire™ (ACQ-5) and the quality of life of patients were monthly evaluated. It has been found that the inclusion of tiotropium bromide in the treatment regimen causes a significant improvement in asthma control, the ACQ-5 index decreased by 48 %. There was also a significant dynamics of postbronchodilation FEV1 and quality of life indicators. None of the patients had a worsening of the disease during the observation period. However, in order to achieve a therapeutic effect, systematic tiotropium bromide therapy was required for at least three months.
