Доброкачественная гиперплазия предстательной железы (ДГПЖ) – распространённое заболевание пожилых мужчин. Трансуретральная хирургия ДГПЖ – «золотой стандарт» лечения, однако методы различаются используемой энергией. Учитывая разные механизмы действия биполярной плазменной и тулиевой волоконной лазерной энергии на ткани простаты, изучение структурных изменений при их воздействии является актуальным.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Изучить особенности структурных изменений простаты в условиях выполнения биполярной плазменной и тулиевой лазерной энуклеаций доброкачественной гиперплазии предстательной железы.</p></sec><sec><title>Методы</title><p>Методы. 31 пациент с ДГПЖ разделены на две группы. 1-й группе (n = 17) выполнена трансуретральная плазменная энуклеация простаты; 2-й группе (n = 14) – трансуретральная тулиевая волоконная лазерная энуклеация. Интраоперационно забирали фрагменты хирургической капсулы простаты, которые обрабатывали по стандартным схемам для исследования методами световой и электронной микроскопии. Учитывали ширину коагуляционного некроза, рельеф линии воздействия, степень выраженности деструкции клеток и межклеточного матрикса простаты.</p></sec><sec><title>Результаты</title><p>Результаты. Исследования показали клиническую и функциональную периоперационную гомологичность сравниваемых методов хирургического лечения ДГПЖ. Данные световой и электронной микроскопии предстательной железы показали большее повреждающее действие биполярной плазменной энергии, что проявляется большей шириной зоны коагуляционного некроза, разорванным и рельефным видом линии рассечения, выраженными нарушениям целостности клеточных элементов и компонентов межклеточного матрикса. При лазерном воздействии изменения проявляют себя в меньшей степени, что свидетельствует о более щадящем действии тулиевого волоконного лазера на компоненты простаты.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные результаты микроскопии свидетельствуют о том, что интраоперационное действие тулиевого волоконного лазера менее травматично для клеток и межклеточного матрикса хирургической капсулы простаты по сравнению с биполярным плазменным воздействием.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Benign prostatic hyperplasia (BPH) is a common disease in older men. Transurethral surgery in BPH is the gold standard for treatment, but the techniques differ in the energy used. Considering the different mechanisms of action of bipolar plasma and thulium fiber laser energy on prostate tissue, the study of structural changes under their influence is relevant.</p></sec><sec><title>The aim</title><p>The aim. To study the features of structural changes in the prostate during bipolar plasma and thulium fiber laser enucleation of benign prostatic hyperplasia.</p></sec><sec><title>Methods</title><p>Methods. Thirty one patients with BPH were divided into two groups. Group 1 (n = 17) underwent transurethral plasma enucleation of the prostate; Group 2 (n = 14) underwent transurethral thulium fiber laser enucleation. Fragments of the surgical prostate capsule were collected intraoperatively and were processed according to standard examination protocols using light and electron microscopy. The width of coagulation necrosis, the relief of the dissection line, the severity of cell and the prostate intercellular matrix destruction were taken into account.</p></sec><sec><title>Results</title><p>Results. Studies have shown the clinical and functional perioperative homology of the compared methods of surgical treatment of BPH. Data from light and electron prostate microscopy showed a greater damaging effect of bipolar plasma energy, which is manifested by a larger width of the zone of coagulation necrosis, a torn and raised appearance of the dissection line, and pronounced integrity violations of cellular elements and intercellular matrix components. Laser exposure causes less pronounced changes, which indicates a more gentle effect of the thulium fiber laser on the prostate components.</p></sec><sec><title>Conclusion</title><p>Conclusion.
