Когнитивные нарушения являются одним из важных факторов инвалидизации у лиц пожилого и старческого возраста. Их роль в прогнозе наиболее ослабленных маломобильных пациентов требует уточнения. Цель исследования. Оценить влияние исходного когнитивного статуса в группе патронажных пациентов в возрасте 60 лет и старше на риск смертности в течение 1 года.Материалы и методы. В исследование были включены 450 пациентов, находящихся под наблюдением патронажных врачей ГБУЗ «Диагностический центр № 3 ДЗМ» (г. Москва), с наблюдением в течение 1 года (с июля 2019 г. по июль 2020 г.) и оценкой числа выживших через 1 год. У всех пациентов была выполнена комплексная гериатрическая оценка с анализом исходного соматического, функционального, нервно-психического и социального статусов. Для скрининга когнитивных нарушений была использована Краткая шкала оценки психического статуса (КШОПС), с выделением подгруппы пациентов с деменцией (КШОПС ≤ 24 баллов) и недементными когнитивными нарушениями (КШОПС &gt; 24 баллов).Результаты. Из 450 пациентов при включении в исследование деменция выявлялась в 44,2 % случаев. Пациентов с деменцией отличали большая выраженность болевого синдрома, нарушений сна, депрессии, мальнутриции и анемии. Через 1 год в группе пациентов с деменцией скончалось 34 из 196 пациентов (17,3 %), оставшихся под наблюдением, а в группе пациентов с недементными когнитивными нарушениями – 18 из 248 человек (7,3 %; р = 0,002). По данным многофакторного анализа, независимыми факторами риска смерти у пациентов с деменцией являлись анемия, нарушения слуха и переломы костей в анамнезе.Заключение. Оценка когнитивного статуса ослабленных маломобильных пациентов важна с целью определения наиболее уязвимых лиц с высоким риском неблагоприятных исходов.</p></abstract><trans-abstract xml:lang="en"><p>Background. Cognitive impairment (CI) is one of the important disability factors in the elderly. The role of CI in prognosis of the frailest patients with limited mobility is uncertain.The aim. To determine the influence of the initial cognitive status in the group of patronage patients aged 60 years and older on the risk of mortality within one year.Materials and methods. Study group consisted of 450 patients from patronage group with one-year period of observation (from July 2019 till July 2020). Initial physical, functional, neuropsychiatriac and social statuses were evaluated by comprehensive geriatric assessment. Cognitive impairment was screened by Mini-Mental State Examination (MMSE), with dividing patients into dementia group (MMSE ≤ 24) and no-dementia group (MMSE &gt; 24). The mortality rate after one year was assessed.Results. Of the 450 patients included in the study, dementia was present in 44.2 %. Patients with dementia were more prone to greater severity of chronic pain, sleep disturbances, depression, malnutrition and anemia. After one year of observation 34 out of 196 patients (17.3 %) in the group of patients with dementia and 18 out of 248 patients (7.3 %; p = 0.002) in the no-dementia group died. According to multivariate analysis, independent risk factors for death in patients with dementia were anemia, hearing impairment and a history of bone fractures.Conclusion.
