По данным литературы, развитие синдрома смежного уровня через 10 лет после задней ригидной стабилизации отмечается у 6,7–80 % больных, из которых у 24 % возникает необходимость ревизионных операций.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Провести комплексный анализ результатов хирургического лечения пациентов с дегенеративными заболеваниями поясничного отдела позвоночника с использованием ригидных систем фиксации.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено мультицентровое ретроспективное исследование результатов хирургического лечения 268 пациентов с дегенеративно-дистрофическими заболеваниями поясничного отдела позвоночника, которым было проведено моно- или бисегментарное декомпрессивно-стабилизирующее вмешательство с выполнением методики TLIF (Transforaminal Lumbar Interbody Fusion) и открытой транспедикулярной ригидной фиксацией. Исследование включало рентгенографию, магнитно-резонансную и компьютерную томографию (в двухэнергетическом режиме) межпозвонковых дисков и изолированной фасеточной дегенерации верхнего смежного уровня.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Сочетание исходной дегенерации смежного позвоночно-двигательного сегмента в виде дегенерации диска III степени и выше по Pfirrmann с пороговыми значениями плотности наружной хрящевой пластинки от 161,7 ± 18,8 до 164,8 ± 14,2 HU, плотности наружной фасетки – от 702,43 ± 12,3 до 713,65 ± 13,6 HU и плотности внутренней фасетки – от 580,5 ± 11,6 до 582,1 ± 15,1 HU, а также с признаками сегментарной нестабильности смежного сегмента можно рассматривать как факторы риска развития его дегенерации. При прогрессировании дегенерации верхнего межпозвонкового диска в сроки 12–60 месяцев после операции проводились ревизионные вмешательства.</p></sec><sec><title>Заключение</title><p>Заключение. Проведение бисегментарной стабилизации с признаками исходной дегенерации в межпозвонковом диске и фасеточных суставах в верхнем сегменте доказало свою клинико-инструментальную эффективность в сроке 36 месяцев, позволяя снизить риски развития дегенеративного заболевания смежного диска.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. According to the literature, the development of adjacent-level syndrome 10 years after posterior rigid stabilization is noted in 6.7–80 % of patients, of which 24 % require revision surgery.</p></sec><sec><title>The aim</title><p>The aim: to conduct a comprehensive analysis of the results of surgical treatment of patients with degenerative diseases of the lumbar spine using rigid fixation systems.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A multicenter retrospective study of the results of surgical treatment of 268 patients with degenerative-dystrophic diseases of the lumbar spine, who underwent mono- or bisegmental decompression-stabilizing intervention with the implementation of the TLIF (Transforaminal Lumbar Interbody Fusion) technique and open transpedicular rigid fixation, was conducted. The study included radiography, MRI and CT (in 2-energy mode) of intervertebral discs and isolated facet degeneration of the upper adjacent level.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. The combination of the initial degeneration of the adjacent spinal motion segment in the form of disc degeneration of grade III and higher according to Pfirrmann with threshold values of the density of the outer cartilaginous plate from 161.7 ± 18.8 to 164.8 ± 14.2 HU, the density of the outer facet – from 702.43 ± 12.3 to 713.65 ± 13.6 HU and the density of the inner facet – from 580.5 ± 11.6 to 582.1 ± 15.1 HU, as well as with signs of segmental instability of the adjacent segment can be considered as risk factors for the development of its degeneration. With the progression of degeneration of the upper intervertebral disc, revision interventions were performed within 12–60 months after surgery.</p></sec><sec><title>Conclusion</title><p>Conclusion.
