Важными звеньями патогенеза холодовой травмы являются повреждения сосудов и мощная системная альтерация. Зачастую длительность течения раневого процесса негативно отражается на функции большинства органов и систем, в том числе и почек. Почечная недостаточность у больных с отморожениями развивается в остром периоде холодового повреждения. На это указывают снижение количества мочи и повышение уровня креатинина у пострадавших с отморожениями. В связи с этим выявление и анализ динамики новых маркеров почечной дисфункции у пациентов с отморожениями является перспективным как с научной, так и практической точки зрения.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Исследовать динамику изменений уровня сывороточного креатинина, NGAL и цистатина C у пациентов с местной холодовой травмой.</p><p>Материалы и методы исследования. В исследование включено 60 пациентов с отморожениями III–IV степени дистальных сегментов конечностей. Исследование проведено в зависимости от объёма поражения и сроков с момента криоальтерации.</p></sec><sec><title>Результаты исследования</title><p>Результаты исследования. У пациентов с отморожениями III–IV степени в крови выявлено повышение уровня липокалина и сывороточного креатинина. Концентрация липокалина и сывороточного креатинина прямо пропорциональна объёму поражённых холодом тканей. Показатели липокалина и сывороточного креатинина снижаются в поздние сроки криоповреждения. Уровень цистатина С статистически значимо снижается во все периоды отморожений; концентрация последнего не зависит от тяжести криоповреждения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Important links in the pathogenesis of cold alterations are vascular damage and powerful systemic alterations. The presence of premorbid pathology and the duration of the wound process negatively affects to the function of most organs and systems, including the kidneys. Renal insufficiency in patients with frostbite develops in the acute period of cold damage. This is indicated by a decrease in the amount of urine and an increase in creatinine levels in victims with frostbite. In this regard, the identification and analysis of the dynamics of new markers of renal dysfunction in patients with frostbite is promising both from a scientific and practical point of view.</p></sec><sec><title>The aim</title><p>The aim. To investigate the dynamics of changes in serum creatinine, NGAL and cystatin C levels in patients with local cold trauma.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 60 patients with frostbite of the III–IV degree of distal limb segments. The study was carried out depending on the volume of the lesion and the timing from the moment of cryoalteration.</p></sec><sec><title>The results of the study</title><p>The results of the study. In patients with grade III–IV frostbite, an increase in the level of lipocalin and serum creatinine was detected in the blood. The concentration of lipocalin and serum creatinine is directly proportional to the volume of cold-affected tissues. Indicators of lipocalin and serum creatinine decrease in the late stages of cryopreservation.
