У 10–20 % больных метастазы в хориоидею располагаются эксцентрично и не сопровождаются субъективными жалобами. В этих случаях их выявляют случайно при осмотре. В литературе практически отсутствуют сведения о возможности ранней диагностики «малых» очагов метастатического поражения хориоидеи с помощью оптической когерентной томографии (ОКТ).Цель исследования. Выявить диагностические признаки «малых» метастазов в хориоидею.Материалы и методы. В исследование включены 4 пациентки, у которых «малые» метастазы (15 очагов) не были визуализированы по данным ультразвукового исследования (УЗИ).Результаты. Хориоидальные метастазы, не выявляемые по данным УЗИ, преимущественно характеризовались мультифокальным характером роста, были расположены преимущественно вне центральных отделов глазного дна. ОКТ-картина в этих случаях характеризовалась расширением хориоидального комплекса до 1300 мкм с асимметричной формой скатов, неровностями его передней поверхности при умеренно гипорефлективной структуре. Целостность мембраны Бруха при этом сохраняется, но характерно нарушение архитектоники хориокапиллярного слоя при ровном профиле передней поверхности склеры. Вторичные изменения надлежащей сетчатки представлены утолщением ретинального пигментного эпителия, отёком фоторецепторного слоя и на уровне ядерных слоёв, при этом отслойка нейроэпителия (ОНЭ) появляется преимущественно при толщине очага более 400 мкм. Первоначально метастатическое поражение хориодеи можно выявить по данным исследования глазного дна в режиме MultiColor (МС) и ОКТ. По мере появления изменений в надлежащей сетчатке – отёка фоторецепторного слоя с гиперрефлективными включениями и ОНЭ – появляются изменения в картине коротковолновой аутофлюоресценции (BAF, blue-peak autofluorescence): гиперBAF с точечными гипоBAF-включениями, – что приводит к изменению картины глазного дна в режиме МС.Заключение. Всем пациентам с отягощённым онкологическим анамнезом даже при отсутствии зрительных жалоб, помимо тщательного осмотра глазного дна обоих глаз, необходимо проводить исследование глазного дна в режимах MultiColor и коротковолновой аутофлюоресценции с последующим проведением ОКТ в зонах наличия изменений.</p></abstract><trans-abstract xml:lang="en"><p>Choroidal metastasis in 23–30 % of cases have multifocal growth. In 10–20 % of patients do not have subjective complaints, due to eccentrical localization of choroidal metastases. In these cases, they are detected by chance during examination. In the literature there is no information on the possibility of early diagnosis of “small” foci of metastatic lesions of the choroid using optical coherence tomography (OCT).The aim. To identify diagnostic signs of “small” choroidal metastases.Materials and methods. The study included 4 patients in whom “small” metastases (15 foci) were not visualized by ultrasound.Results. Choroidal metastases, which were not detected by ultrasound, mainly characterized by a multifocal growth pattern, and located mainly outside the central regions of the fundus. The OCT picture in these cases was characterized by the expansion of the choroidal complex up to 1300 μm with an asymmetric shape of the slopes, irregularities of its anterior surface with a moderate hyporeflective structure. At the same time, Bruch’s membrane was preserved, but a violation of the architectonics of the choriocapillary layer is characteristic with a flat profile of the anterior surface of the sclera. Secondary changes in the proper retina are represented by thickening of the retinal pigment epithelium, edema of the photoreceptor layer and of the nuclear layers, while neuroepithelium detachment (NED) appears mainly when the focus is more than 400 μm thick. Initially, choroidal metastasis can be detected by the fundus examination in MultiColor and OCT, as changes appear in the retina – edema of the photoreceptor layer with hyperreflective inclusions and NED, changes appear in the blue-peak autofluorescence (BAF) – hyperBAF with point hypoBAF inclusions, which leads to a change fundus pictures in MultiColor mode.Conclusion.
