Проблема ранней диагностики повреждения центральной нервной системы у доношенного новорождённого ребёнка до появления клинических симптомов остаётся актуальной и в настоящее время.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оптимизировать диагностику гипоксического повреждения головного мозга у доношенных новорождённых путём анализа концентрации цитокинов и нейронспецифической енолазы (НСЕ) в пуповинной крови.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В ходе I этапа исследования был выполнен проспективный анализ концентраций таких интерлейкинов, как ИЛ-1β, ИЛ-4, ИЛ-6, ИЛ-8, ИЛ-10, ФНО-α, а также НСЕ, в сыворотке пуповинной крови у доношенных новорождённых детей испытавших внутриутробную гипоксию и/или асфиксию в родах. Во время II этапа исследования произведён ретроспективный анализ клинических данных и дополнительных инструментальных методов исследования. В качестве условного «золотого стандарта», объективно отражающего развитие гипоксического повреждения головного мозга у новорождённых, использовался метод ультразвуковой визуализации – нейросонография.</p></sec><sec><title>Результаты</title><p>Результаты. О развитии гипоксического повреждения головного мозга свидетельствует повышение концентрации ИЛ-1β более 30,3 пг/мл, ИЛ-4 – более 1,7 пг/мл, ИЛ-6 – более 79,4 пг/мл, ИЛ-8 – более 107,7 пг/мл, НСЕ – более 10,3 нг/мл и ФНО-α – более 1,6 пг/мл в пуповинной крови у детей, рождённых на доношенном сроке.</p></sec><sec><title>Заключение</title><p>Заключение. Результаты проведённого нами исследования подтвердили, что комплексная оценка концентрации цитокинов в пуповинной крови позволяет оптимизировать диагностику гипоксического повреждения головного мозга у доношенных новорождённых детей. Оценка данных маркеров непосредственно после рождения позволит оптимизировать тактику ведения новорождённых, перенёсших гипоксическое воздействие внутриутробно и/или интранатально. </p></sec></abstract><trans-abstract xml:lang="en"><p>The problem of early diagnosis of the central nervous system damage in newborn before the onset of clinical symptoms remains relevant at the present time.</p><p>The aim of the study was to optimize the hypoxic brain damage diagnosis in full-term newborns by analyzing the concentration of cytokines in the umbilical cord blood.</p><sec><title>Materials and methods</title><p>Materials and methods. During the first stage of the study, a prospective analysis of concentrations of interleukins (IL-1β, IL-4, IL-6, IL-8, IL-10), TNF-α and neuron-specific enolase (NSE) in the umbilical cord blood serum of full-term newborns was performed. The second stage of the study included the retrospective analysis of clinical data and instrumental research methods. The main method for diagnosing in the development of hypoxic brain damage in newborns was neurosonography.</p></sec><sec><title>Results</title><p>Results. The development of hypoxic brain damage is evidenced by the concentration of IL-1β over 30.3 pg/ml, IL-4 – over 1.7 pg/ml, IL-6 – over 79.4 pg/ml, IL-8 – over 107.7 pg/ml, NSE – more than 10.3 ng/ml and TNF-α – more than 1.6 pg/ml in umbilical cord blood.</p></sec><sec><title>Conclusion</title><p>Conclusion. The results of the study confirmed that the comprehensive assessment of the cytokines concentration in the umbilical cord blood improves the hypoxic brain damage diagnosis in newborns.
