Поражение головного мозга является одним из наиболее распространённых внепеченочных проявлений хронической инфекции, вызываемой вирусом гепатита С (HCV). К настоящему времени установлены его основные клинические и нейрометаболические особенности. Однако морфологическая картина HCV-ассоциированных церебральных изменений и особенности экспрессии молекулярных маркеров вируса в веществе мозга остаются недостаточно изученными, что существенно ограничивает возможности патологоанатомической диагностики рассматриваемого заболевания.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Изучить морфологическую картину и установить особенности иммуногистохимической (ИГХ) экспрессии вирусного маркера HCV NS3 в мозге при хронической HCV-инфекции.</p></sec><sec><title>Методы</title><p>Методы. Исследование выполнено на аутопсийном материале головного мозга умерших в исходе хронической HCV-инфекции (40 случаев) и умерших без признаков психической патологии (15 случаев) с применением комплекса иммуногистохимических и патоморфологических методов.</p></sec><sec><title>Результаты</title><p>Результаты. Показано, что морфологическая картина поражения головного мозга при хронической HCV-инфекции представлена неспецифическими Дистрофическими, воспалительными и пролиферативными изменениями, выраженность которых статистически значимо различается в зависимости от отделов мозга (p &lt; 0,001). Экспрессия маркера HCV NS3 обнаруживается во всех исследованных отделах головного мозга и характеризуется наибольшей интенсивностью в белом веществе больших полушарий и стволовых отделов (р &lt; 0,05). ИГХ-реакция маркера NS3 проявляется в клетках микроглии, эндотелия сосудов, клетках воспалительного инфильтрата и в клетках перивентрикулярной эпендимы. Методом корреляционного анализа установлено, что показатели экспрессии NS3 образовывали наиболее сильные прямые высокозначимые (р &lt; 0,01) корреляционные взаимосвязи с такими категориями, как экспрессия CD68, микроглиоз и аксональная дегенерация, что позволяет рассматривать их как основные факторы, посредствам которых реализуется повреждающее воздействие HCV в мозге.</p></sec><sec><title>Заключение</title><p>Заключение. Изучены типичные морфологические изменения головного мозга при хронической HCV-инфекции. Установлены особенности ИГХ-экспрессии вирусного маркера NS3 в различных отделах мозга. Полученные результаты исследования могут быть использованы для улучшения патологоанатомической Диагностики поражения мозга при хронической HCV-инфекции.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Brain lesion is one of the most common extrahepatic manifestations of chronic HCV infection. To date, its main clinical and neurometabolic features have been established. However, the morphological picture of HCV-associated cerebral changes and peculiarities of viral molecular markers expression in brain matter remain poorly understood. This significantly limits the possibilities of pathological diagnosis of this disease.</p></sec><sec><title>Aims</title><p>Aims. To study the morphological picture of brain lesion during chronic HCV infection and to determine the peculiarities of immunohistochemical (IHC) expression of the HCV NS3 antigen in brain matter.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study was performed on autopsy brain of 40 patients who died in the outcome of HCV infection and 15 patients died without signs of mental pathology. Complex of pathomorphological and ICH methods was used.</p></sec><sec><title>Results</title><p>Results. It was demonstrated that morphological picture of brain lesion in chronic HCV infection is represented by nonspecific dystrophic, inflammatory and proliferative changes, the severity of which varies significantly depending on the brain departments (p &lt; 0.001). Expression of the NS3 marker is found in all studied brain regions and shows the highest intensity in white matter of cerebral hemispheres and in brain stem (p &lt; 0.05). The ICH reaction of NS3 is manifested in microglia cells, vascular endothelium, inflammatory infiltrate cells and in periventricular ependyma cells. Using correlation analysis method it was found that NS3 expression formed the strongest correlations with such categories as CD68 expression, microgliosis, and axonal degeneration. This allows considering they are the main factors by which HCV realizes its deleterious effect on the brain.</p></sec><sec><title>Conclusion</title><p>Conclusion. Characteristic morphological changes of the brain in chronic HCV infection were studied. The features of ICH expression of the NS3 in various parts of the brain are established.
