Анатомичность расположения трансплантата приносят в жертву его изометричности как основополагающему принципу реконструкции связочного аппарата любого сустава. Трансплантату придают более вертикальное положение, дублируя лишь переднемедиальный пучок связки.</p><p>В связи с этим основным механизмом такого совершенствования на сегодняшний день является применение анатомичного трансплантата, по подобию этой связки состоящего из переднемедиального и заднелатерального пучков и ориентированного в косом положении в отношении суставных поверхностей большеберцовой кости. С точки зрения биомеханики каждый пучок сам по себе «неизометричен», но «изо-метрична» связка в целом как комплексная структура. Главным недостатком этого трансплантата является значительное техническое усложнение такой операции со всеми вытекающими из этого последствиями. Сложность заключается в том, что площадь контакта передней крестообразной связки широкая при небольшой площади сечения на протяжении полости сустава. Подробное изучение учёными топографической анатомии передней крестообразной связки позволило объяснить этот феномен, приблизившись к решению проблемы.</p><p>Нормальная анатомия передней крестообразной связки представлена двумя пучками - переднемедиальным и заднелатеральным. В этой связи можно говорить о том, что двухпучковая пластика передней крестообразной связки восстанавливает анатомию до уровня, близкого к нативному, хотя в настоящее время наиболее часто используемой является однопучковая пластика как более простая в техническом исполнении. Тем не менее, консенсус в выборе вида пластики при восстановлении передней крестообразной связки до сих пор не был достигнут.</p><p>В обзорной статье представлены различные взгляды на основные методы артроскопического восстановления передней крестообразной связки коленного сустава на современном этапе развития хирургии. </p></abstract><trans-abstract xml:lang="en"><p>Researches performed in the past 20 years show the necessity of the restoration of anatomic position of anterior cruciate ligament as the main stabilizing structure of the joint in combination with the restoration of other injured elements and correction of deformities, which affect the outcomes of the treatment.</p><p>Complexity of the restoration of anatomic position of torn anterior cruciate ligament is caused by its wide area of contact in combination with relatively small sectional area through the joint cavity. More detailed study of topographic anatomy of anterior cruciate ligament allowed the researchers to explain this phenomenon, which pushed them closer to the solution of the problem.</p><p>Systemic anatomy of anterior cruciate ligament include two bundles - anteromedial bundle and posterolateral bundle. Double bundle plasty of anterior cruciate ligament restored anatomy to its nearly normal condition, but single bundle plasty is used much more often because of its simpler technique.
