Магнитно-резонансная томография не только обладает мощными возможностями по визуализации, но и представляет интерес в плане получения представлений о микроструктурных и биохимических сдвигах в тканях суставов при остеоартрите. Это свойство магнитно-резонансной томографии может быть расширено путём текстурного анализа карт матриц смежности Т2-взвешенных изображений.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: оценить возможности Т2-взвешенных изображений магнитно-резонансных томограмм в диагностике микроструктурных изменений субхондральной кости при остеоартрите на основании анализа спектральной вариабельности времени поперечной релаксации.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Было обследовано 62 больных остеоартритом и 8 добровольцев без остеоартрита. Всем пациентам выполнена магнитно-резонансная томография коленных суставов на высокопольном томографе с напряжённостью магнитного поля 1,5 Тесла. Для оценки МР-изображений использовали полуколичественные измерения тканей суставов на основании протокола WORMS. Для оценки вариабельности времени поперечной релаксации ручным способом производили сегментацию Т2-взвешенных изображений субхондральной кости во фронтальной проекции. Плотность протонов оценивали по 3D-гистограмме по шкале от 0 до 255.</p></sec><sec><title>Результаты исследования</title><p>Результаты исследования. При I стадии остеоартрита отмечается уменьшение интенсивности магнитно-резонансного сигнала по всей поверхности большеберцового плато с минимальными значениями в области медиального отдела коленного сустава. При II стадии остеоартрита было зарегистрировано ещё большее снижение количества протонов, совершивших фазовый переход с наименьшим значением в области медиального отдела. Текстура субхондральной кости при III стадии гонартроза характеризовалась значительным снижением интенсивности сигнала в области медиального плато большеберцовой кости. При терминальной IV стадии остеоартрита наблюдалось значительное уменьшение интенсивности текстурных спектров в области интереса.</p></sec><sec><title>Заключение</title><p>Заключение. Выявленная закономерность изменения спектра времени релаксации Т2-взвешенных изображений отражает дегенеративной процесс в субхондральной кости при остеоартрите. Данное свойство текстуры может быть использовано в оценке микроструктурных изменений субхондральной кости при остеоартрите, в том числе на «ранних» стадиях заболевания.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Magnetic resonance imaging not only has powerful capabilities for visualization, but is also of interest in terms of obtaining ideas about microstructural and biochemical changes in the tissues of the joints in osteoarthritis. </p></sec><sec><title>Aims</title><p>Aims. To assess the possibility of T2-images of magnetic resonance imaging in the diagnosis of microstructural changes in the subchondral bone in osteoarthritis. </p></sec><sec><title>Materials and methods</title><p>Materials and methods. 62 patients with osteoarthritis and 8 volunteers without osteoarthritis were examined. All patients underwent magnetic resonance imaging of knee. To assess the variability of transverse relaxation time, the T2-images segmentation of the subchondral segmentation in the frontal projection was performed by hand. The proton density was estimated from a 3D histogram on a scale of 0 to 255. </p></sec><sec><title>Results</title><p>Results. At the first stage of osteoarthritis, the intensity of the magnetic resonance signal decreases over the entire surface of the tibial plateau, with minimal values in the region of the medial part of the knee joint. At stage 2 osteoarthritis, there was an even greater decrease in the number of protons that made the phase transition with the lowest value in the medial region. The subchondral bone texture in stage 3 was characterized by a significant decrease in signal intensity in the region of the medial plateau of the tibia. In the terminal stage of osteoarthritis. </p></sec><sec><title>Conclusion</title><p>Conclusion.
